We offer a full range of holiday vaccinations and antimalarial medicines. In order to best advise you, we need to know where you are traveling, together with lots of other information. We need time to do this, and have therefore developed a travel health questionnaire for you to complete. Please try to give us plenty of notice about your intended holiday so we can ensure that you are adequately protected. Ideally we would like to discuss your needs with you at least six weeks ahead of your travel date.
If you require vaccinations for travel contact the surgery for a travel questionnaire which we would ask you to complete as fully as possible and return to the Practice at your earliest convenience.
If you have not already done so, please make a 10 minute appointment to see the Practice Nurse. Please allow 10 working days in between returning the form and visiting the Nurse. This will ensure that we have reviewed the information provided, researched the most current recommendations for the countries you intend to visit and have the relevant vaccines available for your appointment.
Please note that whilst most of the vaccines for travel are covered within the scope of the NHS services, some do attract either a charge or Private Prescription e.g. Yellow Fever, Hepatitis B, Swine Flu and Malaria.
Should you require any further information, please do not hesitate to contact us.
