Prescription Requests

Please note that as from 1st July we will no longer be taking prescriptions over the phone. This will prevent dangerous errors being made and leaves the telephone lines free for urgent matters. This does not apply to housebound patients.

How to Order your Prescription

Please use one of the following methods – please always remember to let us know if you want your prescription to go to a specific pharmacy or if you wish to collect this.

In Person – you can order in person by returning the right hand half of a previous prescription for the required medications or by submitting a handwritten request. These can be handed into the Practice or posted through our letterbox.

Online – DalryHealthCentre_Prescriptions@aapct.scot.nhs.uk

By post – you can post your prescription slip or handwritten note to us at the Practice. You must include a stamped addressed envelope if you wish your prescription to be posted out to you. Please allow extra time for any possible delays by the Postal Service.

Pharmacy Service – if you have a preferred pharmacy, you can hand in your repeat prescription request to them.


NHS Ayrshire and Arran has agreed to move to a standardised time frame for ordering of repeat prescriptions. Our clinicians prescribe and dispense around 9 million prescription items each year. This seven day turn around will ensure they have the required time to undertake necessary checks to ensure the safe prescribing and dispensing of your medicines. 

This change will commence on Monday 20th February 2023. We are asking for support from our citizens to do the following:

  1. Please now allow seven days from initially ordering your repeat prescription to collecting your medication at your community pharmacy
  2. Order all of your required prescription items at the same time
  3. If you are on regular medication for a long term condition ask your GP practice or community pharmacy if a serial (longer term) prescription would be suitable for you

Pharmacies offer a prescription collection service from our Practice. Please contact the pharmacy of your choice for more information if you wish to use this service.

We now have a prescription mailbox which is the preferred method when ordering – prescription requests can be emailed directly to the mailbox which will be checked on a regular basis. Please include your name and date of birth, list of medications required and which chemist you would like your prescription to be sent to. Alternatively, you can request your prescription by phoning the Practice between the hours of 10:00am and 12:00pm and 2:00pm and 4:00pm. You can also hand your repeat prescription into the Practice or post through the letterbox.

Hospital Fit Notes

If you are off work due to a Hospital admission, a Sick Note can be issued by the Hospital to cover the period of time you were in the Hospital.  Once discharged, your GP can issue a Sick Note, based on your Hospital discharge letter.  If you think this is your situation, our Reception Team will assist you.  

Please note a Sick Note can be backdated; please discuss this when you speak to your GP/ANP.  Emergency or urgent appointments should not be used to request Sick Notes. 

Self-Certificate

If you have been sick for more than four days in a row, but less than seven, you can self-certify your illness using a SC2 form.  You can obtain this form from your employer or the HMRC Website .

More than seven days off sick

If you're off work sick for more than seven days your employer will normally ask for a Fit Note from your GP or Hospital Doctor. 

How to count sick days

When you work out the number of days that you've been sick, you need to count all the days in a row you've been sick, including days you don''t normally work such as weekends and bank holidays. 

Requesting a Sick Note

If you require a fit note, please email the prescription mailbox using the email below. Please include your:

1) Name 

2) Date of Birth

3) When the Fit Note should be dated from and how long you require the fit note to run.

4) Reason for the Fit Note.
